The research theme 'aging & chronic conditions' focuses on both the prevention of functional decline and adverse health outcomes of chronic disease patients and older people who are living independently in the community.

The primary aim of the TASTE (TAilored Self-managemenT & EHealth) research line is to increase efficiency and (cost)-effectiveness of self-management support in chronic disease patients. This international research collaboration systematically fills the knowledge gap on (non)compliers and (non)responders, on determinants of self-management capacity and facilitators and barriers of self-management support. Thus providing knowledge for subsequent development of tailored nurse-led interventions, including e-health, based on individual patient profiles and preferences aiming at higher effect sizes.

This research program focuses on improving (nurse-led) care that contributes to maintain independence and self-sufficiency. The main target population are those older people and their families who are living independently in the community who have multiple complex care needs due to the existence multimorbidity. We aim to develop successful preventive primary care interventions that are targeted to those who are likely to benefit from it, and that are tailored to the individual health care needs of older people.
